Seems like I've answered this question a time or two before.

One of my favorites early on in my career: Called a three second violation...on a defensive player.

More than one occasion (also pretty early on), blew whistle, indicate two shots to my partner, proceed to reporting area, completely forgot who I called the foul on. DOH!
