I am replying based on some info on the old officiating.com site on over and back and the NBA rule.
The NBA rule you quote is correct, once in the front court, the ball must be passed into the backcourt. On all backcourt violations the NBA brings it in from the NEW front court and the mid court line.
The NBA backcourt is slightly different than NF.. on a pass the ball attains front court status when it breaks the plane. ( Makes it easier to call on a 10 second count than waiting for it to touch on a long pass) and there is no "3 points" (both feet and the ball) for the dribbler.

The foul you are referring to is one that is also different. When a foul occurs on the throw-in but before the ball is released the penalty is a two shot foul wheteher or not there is a penalty situation. In the last two minutes it is one shot plus the ball. If a foul happens before the throw in, it is assumed you are not making a play for the ball... That's off the top of my head. I'm sure Drake will help if this was incorrect
