Mechanic for Free Throw substitution

When a team subs for the free thrower and the free throw is made, what are the proper mechanics. Do you blow the whistle and bring in the sub? Also do you then administer a spot throw-in as opposed to letting the team run the baseline? If so that seems to be a disadvantage to the throw in team. Thanks - I am new to this forum and to officiating and appreciate the help.
