Great question!!!
Don't have rule book with me. After reading the replys I agree that it would VERY difficult to do, but I believe it could be done in a rare occasion. Example: Coach with no T's throws a chair onto the court (i.e. Bobby Knight). I think that would deserve an early exit without warning.
