This Friday night, for the first time, I have been assigned as the referee in a 3-man crew in a HS Varsity game. I know that I have the responsibility to make out the cards to present to the coaches. Is there a standard card that can be used or do most officials make up their own?
As far as I know, the date, home and visiting team names, referee name, and the two umpire names are to be included on this card. Is there anything that I am overlooking?

In my state all we have to do is fill out our name and our ID number on the official scorebook. Now this is for rating purposes. We do not have to give out our ID number at any lower level games. Ratings are only used for varsity games so they can be used for post season consideration.
What you are describing might happen in other places but I would not assume it takes place with most of us. I know the ADs probably have that information so we get paid, but we do not give it directly to the coaches. Peace

No such thing where I live, the R simply signs the book and writes U1/U2 name in it.
