Even if TV is there, then the sub needs to have had reported by the warning horn - whenever the heck that is (usually at some point between 5-6 minutes after the 2nd half horn).

Coach could cuss out the official. Earn a T. 2 FTs (live ball/dead ball), but then you'd resume the game with the OT jump ball.

Well, this is the official rule that applies as per the NCAA. I got the email this morning.
