NFHS Rule

Team A has 6 players. During the 3rd quarter A12 picks up his 5th foul and leaves the game. With 3 minutes to go in the 4th quarter A22 is fouled by B34during a shot and is awarded 2 free throws. A22 then pushes B34 and A22 is called for a T foul. That is A22's 5th foul. Who will shoot the 2 free throws for the foul by B34 on A22.

NFHS: One of the four remaining Team A players in the game, as selected by the Team A coach.

8-2: The free throw(s) awarded because of a personal foul shall be attempted
by the offended player. If such player must withdraw because of an injury
or disqualification, his/her substitute shall attempt the throw(s) unless no
substitute is available, in which case any teammate may attempt the
throw(s) as selected by the team captain or head coach.

Just make sure that it's the "speaking captain" as identified by his jersey number in the pregame meeting, not just any old captain.
