need help with finding something in rulebook

Hey guys, I am the lead official in a 3 man crew for a JV doubleheader tonight. As part of my pregame I want to cover proper throw-in administration, as this has been an area of weakness for the non-varsity officials in my area. What I'm basically meaning is where to take the ball out of bounds after a foul or a violation. I've seen guys in my area calling a foul at the top of the key, yet taking the ball out of bound on the endline.

I know I've seen in one of the NFHS books, either the rule book or an officials manual, some kind of diagram that shows throw-in spots in relation to where a violation or foul happens. Can anybody point me to this, as I'm having trouble finding it in this years books. I'd like to get a copy of that diagram and print it out for my 2 officials tonight, who are both 1st year officials. If anybody can help me out, I'd appreciate it, thanks.
