HR hitter injured during swing?

Score 1-1, top of 9th, R3 and R1, 2 out. Batter severely twists his ankle and lays in the batter's box after hitting a home run. Athletic trainer is standing nearby. How do you handle it? Do runs count?

Similar play in MLB game, the batter shook off the injury and ran around the bases. See video here.

But what if he couldn't get back up after 2 minutes?

Allow a substitute to complete his award.

Same deal if a batter is HBP and is incapacitated. Sub may complete the award.

Agreed with both above.

And don't allow this ridiculous softball play that gained so much press four years ago:
