1. For intentionally removing helmet during a live ball:

This is a delayed dead ball per rule 5-1-2e.

The penalty is listed in Rule 1-4-5.
1st offence = Warning
2nd offence is ejection of offender.

Dead ball/Delayed dead ball chart on pages 36-37 of NF rule book is an excellent resource to look at.

2. For intentionally removing helmet during dead ball:

No penalty.

3. Accidental loss of helmet during live ball:

No penalty. However according to Case book 1.4.5 Sit. F helmets worn should fit reasonable well in order to provide the proper protection for the wearer. This means that if a player wears a helmet whether so large or too small that it will not stay on that person is required to either get a better fitting helmet or secure the helmet (chin strap, tape, etc) so it does not come off. If they refuse they may not play or be on the field during a live ball.
