Although not specifically stated in the 5 man mechanics instructions, the Referee is responsible for "B" after the snap: (1) Watch for fumble by the holder, which would seem to place him in the appropriate position, and focus, to observe the holder's knee if remaining on the ground when passing (forward or backwards) which would cause the ball to be IMMEDIATELY declared DEAD
