Just saw the OP. Here's how I understand it:

If you're calling a delay-of-game violation, it's a warning, then a team T, then another team T, etc. The four warnings are limited to plane violations, spilling water on the floor during/after a time-out, touching the ball after it clears the basket, and huddling before a free-throw. In the last two, I look for an actual delay. (A unintentional tap or quick hand-slap with the shooter is nothing to me.)

If you're calling a delay-of-game foul, it's a technical foul to the player/coach. If it doesn't fall within the four warnings, it has to be a foul.
