OBR 6.07(b) says to nullify any advance or score, but is silent about outs. In the 2012 MLBUM, I don't see anything about BOO, although I may be missing it. But on page 72 of the 2014 BRD, CC cites an official interpretation in the PBUC: "When an improper batter's action causes a runner to be out (a force or double play, for example), if the batting-out-of-order appeal is upheld, only the proper batter is out; any other outs are canceled."

CC also gives an R1, 0 outs, 6-4-3 DP, with R1 returning to 1st. Same result under NCAA.

Beats me why a coach would appeal in these circumstances, giving up an out.
