I disagree with this part. If the T was before the horn ending the third period (as was the Intentional foul), then the T was part of the third period. Handle it the same way -- shoot the free throws, forget about the throw-in and start the 4th period normally.

If the T was after the horn, then it was part of the 4th period, and you are correct.
