I would offer the options for each of the fouls, though they can accept just 1 penalty for multiple live-ball fouls.
  1. Result of the play (spot where ball left field)
  2. 5 yards and rekick (kick OOB)
  3. 10 yards and rekick (IBB during kick)
  4. 25 yards from previous spot (kick OOB option)

This order is calculated to provide the option most likely to be accepted last.
