I agree with your positioning with 2 exceptions:

1. As already mentioned, the new NFHS mechanic is to have T tableside for free throws.

2. I do not stand at the sideline, but rather 1/3 to 1/2 the way between the sideline and the lane, and a step above the FT line extended to get an angle on the 2 lane players who are mine.
