I didn't see the play but it's reasonable to assume both players committed PI penalties on the same play. Since restrictions begin against A at the snap, the receiver could block the defender before the ball is thrown to gain separation...flag 1. The defender recovers and the ball is underthrown with the defender making contact and no attempt to play the ball...flag 2. It's not common, but it is entirely possible.
