For NFHS rules,read the first line of Rule4-19,then read Rule 4-19-2,6&12.For NCAA rules,read Rule 4-9-1&2.Both rulebooks state exactly what you're looking for-it's charged to the player and the team.
Don't know or care about NBA rules.
