I read comments from the NCAA a few years ago on this subject. I think it was in Sport Magazine, but I'm not sure. The decision to show which games in which areas are a combination of three things (in this order):

1) geographical proximity of the venue
2) balancing of markets to give regional advertisers an equal representation
3) requests from local affiliates

Back when NFL telecasts were more regionalized, I think they used the same formula, at least that's what someone from one of the local stations told me.

Conspicuous by its absence is the criteria "What the fans want".
