It makes no difference where it "originated." An elbow foul which makes contact above shoulder level results in an ejection. NBA Rule 12, Section 5 l.(3)
The contact was made right at shoulder level then Miller's head snapped and he got hit in the chin. Either way i would eject him because it was as a retaliation blow.
