DELAY warnings are per team, per set. (And, I should add, CONDUCT cards are per person, per match). So, one in the third and one in the fourth would be two yellows. But, for repeated delays, you can go to a conduct card. See the NOTE at the end of 9-9.
That said, illegal libero replacement after the whistle for serve is a loss of rally/point. See 10-4-4 penalty. (This will change next year)
