Most states there is a set fee assigned by the state association for contests at each level. States like Iowa it is done per district and there are even a few states that the schools negotiate with each ref (contractor) individually. Obviously mileage/travel is a moving target, but I tried to capture the most information with not too much clutter (3-man lower level is rare and can probably be ignored).
Ultimately, what matters is how much the official actually receives for doing the game, regardless of all of the methods of determining that amount. Seeing a $20 difference between states doesn't mean much if the lower state pays $20 in travel while the higher state doesn't pay travel. Compiling a list of the only one term in the equation is like predicting who will win a basketball game by calculating how tall the starters are.
