Fed rule 8, Section 4, Article 3 - runner entitled to advance.... This is covered by the same sub-article (don't remember which one) that deals with a batted ball landing in fair territory and the bouncing out of play. You are correct, it is two bases from the time of the pitch. Roger should be able to cite the sub-article when he sees this, otherwise I'll bring my book in from the van tomorrow.
If ,however, your judgement is that the defender intentionally put the ball out of play, it it two bases from the time the fielder threw/carried the ball out of play.
