Suppose ASA bans the "Blasto Killer Mark VI" bat, and then somebody gets one professionally repainted so that it looks exactly like the "Milquetoast Marshmallow Model 1," which is on ASA's list of legal bats.

Would this be considered an altered bat?

Also, a bat is considered altered if a cone- or flare-shaped grip has been attached to it. But are there any ASA-legal bats that are manufactured with a cone- or flare-shaped handle?

No, it is a banned bat. An altered bat is a piece of equipment which was at one point legal. But, how are you going to prove it is a banned bat?

If this isn't caught in the pre-game bat check, it would be an altered bat. This is specifically covered in Rule 1-Altered Bat. The answer to the last question is obviously no.
