Good question and one that my crew of rookies approached me with this past week...

after discussing with my U (27 yr veteran and NCAA R) this is what we came up with...
1) The "assist" by A is generally called only when A is in the act of pulling or carrying the runner. In the case you provide, the action by A usually results in the pile collapsing, therefore A gains no advantage.

2) The block in the back is ignored using the advantage/disadvantage philosophy stated in 1 above.

Other regions may have different views and I would like to hear them...
