During a PAT-
1. the defense jumps offsides (officials blow the wistle a little bit late)
2. the defense roughs the kicker

What are the options?
What I did was 1. penalized the defense for offsides on the pat
2. penalized the defense for roughing the kicker on the kickoff

I beleive both can be penalized on the PAT, but only penalty 2 can be enforced on the kickoff..

In NFHS, once the defense enters the neutral zone the play should be blown dead, then you can't have roughing the kicker, only a PF, late hit.

The NF rule change allows penalties on scoring plays to carry over to the succeeding spot. The only time that would be the kickoff is on successful field goals or successful tries.

NCAA rules allow personal fouls to carry over to the kickoff.

Under NCAA what you described is correct. Under NF both dead ball penalties would be enforced on the try.
