When working 3-whistle, there are always 2 officials on the side of the court where the throw-in is being made. So in #1, the L slides over to the old C's side. The old C becomes the T and administers the throw-in. The old T slides up to the FT line and becomes the C.

In #2, you are correct, and it's handled just like #1.

In #3, you are also correct.

I should say that I'm answering according to NCAA mechanics, b/c all my HS games are 2-whistle. But I'd be surprised if Fed mechanics were different.
