Following the team warning for delay, commit a violation of the throw-in boundary-line plane.
PENALTY: Two free throws plus ball for division-line throw-in.

Are there any other situations, by rule, where an official would issue a warning for delay before the "T"?

Could a Team receive 3 warnings for delay, one for each situation above, before receiving a "T"?

Yes, there are three SEPARATE delays you only give a "T" when the SPECIFIC delay is violated more than once.
