Actually, there are four elements. What you have listed as number one is really two different things. And, what you have as number three is not exactly correct. The four are:


1)there must be team control
2)the ball must achieve frontcourt status
3)the team in team control must be the last to touch in their frontcourt
4)that same team must be the first to touch after the ball has been in the backcourt

As you can see, there is no requirement for “team control in the frontcourt.” Also – since there is no team control during a throw-in, you cannot have this call during one.
