Well, based off the info in the OP- the basket counts, T official notifies Coach B of the 5th personal, the player of the 5th personal, and tells the scorekeeper to start the 20 second replacement clock. and resume play as normal

I'm wondering about this too. I typically post here only when I have a question about keeping the book or when I've made a mistake keeping the book that I want to ask about. However, I've never notified the officials when a player had 4 fouls (though I try to keep the coaches informed). Is notifying the officials about a 4th foul something else I should be doing? Thanks.

No. i don't care, and it's as likely to cause momentary confusion as anything.
