I don't know about the NBA review. I think they only review if a last second shot (at the end of a quarter or OT) was released in time. I don't know if they use it to determine who came off the bench in a fight situation.
All the NCAA uses are found in Rule 2 and are online at http://www.ncaa.org . There are too many to list in a post.
