Wow - great list (and most apply to FIBA as well!).

One that I have had a lot recently - a playing driving to the basket extends their arms to shoot the ball - and causes contact with a defender that has their arms straight up. Obviously no foul - but players and coaches complain that "the refs aren't protecting the shooter". Has been really getting on my nerves lately.
