I heard recently retired NBA referee Nolan Fine speak at a camp, and he said that NBA referees and coaches always address eachother by first name. I found that intresting.

60 refs + 30 coaches = pretty easy thing to do.

I've seen this often at the D-I level as well, particularly with the 'big-name' coaches and refs.
