Why not just use the one on pages 12-16 of the NFHS Officials Manual?

Go to the forms section of our association website at http://home.officiating.com/scbowbo/referfrm.html

Scroll down to the section called "PREGAME CONFERENCE FORMS"

Select the one that says "Three-person Pregame Form (Word Doc)"
