I am going to be making a new payment slip for our association, and I am in need of idea's does anyone have one you could send to me?

You mean something with a spot for the ref's name, the date of the contest, the contestants, the amount to pay, the address to send it to, a phone # and a social security number?

Nah...can't think of anything. Sorry.

Our association has 2 types of games cash and charge.

The cash games are just that. The officials get payed sometime before they leave the site.

The charge games involve our association sending an invoice out to all the leagues and schools we service, and they pay the association who in turn writes us, the officials, a check at our monthly meetings.

Why would you give your social insurance number to a coach or AD???

Just picking on ya B.

Pretty much everything I gave you is standard stuff, sometimes there's other information that might be needed for your records (like game number or number assigned to the official).

If your form needs to have multiple games per pay sheet that are turned in weekly or monthly you might need to think more about how to best format it.

Good question! Actually here in Illinois, on the contracts we sign it asks for our SS #. Not a big fan of that!

Here in MI, cash is only paid for non-HS games on occaison.

Most HS/MS games get paid by check by the school and you DO have to give your SS# in advance. Other times the assignor (association) cuts the check and you need to provide them with your SS# as well.

If you go to the forms section of our website at http://home.officiating.com/scbowbo/referfrm.html, page down until you get to the form entitled, "REFEREE PAY SHEET FOR GAME FEES AND MILEAGE."

Feel free to download that word doc to your PC and modify it for you own use.

There is also an EXCEL spreadsheet that automatically calculates the game fees and mileage for all our officials on that page... If you're an EXCEL expert, you can modify that one for your use too.

Seriously (literal and figuratively), It is my understanding that the organization who offers the contract is using SSNs to file your earnings with the IRS.

correct me if im wrong, but your SS Number is needed by schools in case you work alot and make at least 600.00 in a given year, then by our wonderful tax code LOL they are required to submit a 1099 even though here in IL we are independent contractors.

You are correct. If they request it, you have no choice. You must give it to them.

I find that 95% of the games I do are "cash games"-very few HS don't pay us the night of the game.

We have to fill out a district provided pay slip and they mail us a check. It can take anywhere from a week to a month to be paid. We do have to put our SSN # down. We have been told that they will submit it to the IRS if we make $500 or more from that district. Some districts also require you to register with them as a vender and then you also have to have a vender #.

I've don't think I have ever given my SS # on a contract -- varsity in soccer and everything else in b'ball and v'ball.
