Put it over a lightbulb - per IRS regulations, he's required to write that in in invisible ink. It's there.

Or, you could just give him a call and see what he used last year. Or (if you had enough time - perhaps after filing a 4868 - request for extention) you can get a copy of previous years' tax returns from the IRS.
