Inadvertent Free Throw

So here is the situation. B21 picks up his 5th foul and the C official reports the foul to the table. Table notifies C that B21 has 5 fouls and C notifies head coach. As head coach is figuring out who to put into the game, lead official administers the basketball to A32 who shoots and makes the first free throw of two. B head coach replaces B21, and wants the free throw to not count as he had not properly replaced B21 prior to lead official administering the basketball. Crew decides that there was no harm and counts the free throw. What should have happened other than of course the lead official making sure everyone was a go for the free throws and not administering the basketball to A32.

In my opinion, not much else to do other than the reporting official has some responsibility to let the lead know why we are stopping the game. Correct in counting the free throw as no correctable error was made.

After getting it wrong, the officials then got it right.

There's a case or an interp on this -- it was discussed here within the past couple of weeks.

The C needs to notify his crew before notifying the coach. I pregame it every single game because so many officials fail to communicate properly in this situation.

As far as the resolution, it's not a correctable error. This question comes up 2-3 times every season.

Odd. I don't get it. Does this really happen as often as the questions about this situation that appear on the Forum? This question should come up once, or maybe, twice every five years. What's the rush to get the free throw taken? Don't most of us make eye contact with our partner before administering a free throw? And maybe make sure that we're on the same page: basket counted, or didn't count, number of free throws, shooter, clear lane and identify throwin spot for intentional, and technical fouls, etc. How can just the administering lead, in a two person crew, observe all ten players, on the lane, on the line, behind the arc, for violations? And possibly beckon in substitutes?

Honestly, I see this a handful of times a season. Guys get in a hurry and anything outside the routine is a disruptor. Not saying it's right, by any means, but it happens far more than it should.

In fact, I saw it last night in a HS playoff game here. Center gets a good shooting foul, takes it to the table, and warns the home HC after the report for his response. While he is still administering the warning, the first FT went up. Sigh.

How often is it the referee dropping the ball, and how often the table? I recall seeing this once this season, and it was the table not telling the refs until after the first FT.

This situation happened to me once this season. I reported a foul. The table had a question (I cannot remember what it was but there was a slight delay). When I turned around to see the floor, the first free throw was finished. The table then lets us know that it was the players 5th foul. The coach who's player fouled out wanted a "do over" on the made free throw. I simply told him no and we moved on and no one said a word.

I can certainly see this happening with a three person crew, but I officiate in Connecticut, where three person crews are as common as teeth on chickens.
