I have been officiating basketball for a few years and I have a pregame sheet that I use with my partner to go over important points before the game. Does anyone know of a similar form for baseball (OBR)? Also, what would you consider the necessary points that need to be covered in a pregame? (Two man)

You can get a small one from +POS or Gerry Davis Sports. Look for the info cards or any kind of pad that you keep your lineups in. Usually they have some kind of pregame information on them. I do not know of any that are OBR specific, which I really do not know why that is important. You are the one that decides what you talk about, the info cards give you some bullet issues to cover (infield fly, fair/foul ball coverage).
