What happens if the catcher sets up outside the catcher's box? Do the restrictions of staying inside the catcher's box during an intentional walk prior to the release of the pitch also apply to any pitch made by the pitcher? Is the penalty the same as on an intentional walk, a balk? What if there are no runners on base?
