The catcher may not set up outside the box. First I am going to do some preventative umpiring. If I am plate umpire I will put my hand up in the do not pitch signal and instruct the catcher to get inside the box before I will give the play ball signal.
Rule 1-1-4: At the time of the pitch the catcher must be in the catchers box.

2-28-3: Time of the pitch varies according to windup or set position. Read this rule diligently as int is important in so many applications.
If the catcher vacates the box or is outside it at the time of the pitch the ball is dead immediately. Assess the appropriate penalty.
Penalty: NO runners = Illegal pitch (2-18-1). The batter is awarded a ball.
Penalty: Runners on = Balk (2-18-1). Each runner is awarded one base.
The restrictions apply on all pitches no matter the situation.
Of course in FED ball there is no need to pitch to a batter for an intentional walk. just send him to first base when defense notifies you of their intentions.
