Bases are loaded with two outs. The batter hits a grand slam (over the fence), but misses second base while running the bases. After the play is over the umpire calls the batter out for missing the base. How many runner score?

Three. Had BR missed first base, no runs would score.

Doesn't there have to be an appeal play for the out to be called?

I assumed either:
a) This was a game under Fed rules, or
b) The defense did appeal.

The question wasn't about appeals, but about scoring.
