when I work, if there is a true reason, he goes out and sub comes in for him. If there is a point where he can come back in, he comes in for the sub. In our league, all players must play 2 innings. Therefore, player A has to sit at least 2 innings.
The only exception I have ever made was this sitch - B1 gets hit with fastball in gut. He goes to bench. His turn at bat does not come up again that inning, but he is ready to go at the half-inning. I send him in and call it a pinch runner situation. Probably not the best call I ever made, but that is what I ruled.
