Batter swings on bounced ball???

Hoping for some help on this one. If the pitch hits the ground before the plate and it takes a bounce, what is the call if the batter swings on it? Ball or strike? What if the batter hits it fair or foul for that matter?

1. Swings and misses...strike.
2. Swings and hits fair....hit.
3. Swings and hits foul....foul.
4. Watches it go by....ball.

And swings to create a foul tip... on third strike with two out?
Bounce, swing, contact, directly into catcher's mitt, all runners are going. Batter is out or can he run?
