first baseman

OBR:
Runner on first, does the first baseman need to be entirely in fair territory?

Under OBR, yes -- but it's only enforced if someone brings it to the attention of the umpire. The penalty is a "don't do that" -- not a balk. Eject F3 if he refuses to comply.

Fed: No.

What advantage would the team in the field get from being allowed to have fielders other than the catcher in foul ground?

It can be easier / quicker for F3 to make a tag.

In some codes, the rule also applies to all plays -- so you can have someone back up a pickoff or an appeal.

Oh, of course, a left-hander, thanks. A lefty could make such a play forehand instead of backhand if he could stand on the foul side of the base.
