The play has to start from scrimmage, yes.

Yes, as long as the kick originates from on or behind the line of scrimmage. If the pass was complete, the receiver can still attempt the kick from behind the line of scrimmage. If the ball hits the bar or post, it can still score the goal if it goes thru, but is dead in all other respects.
If an opponent collides with the kicker after he kicks, on a broken play there's no roughing-the-kicker liability if it didn't look like he was going to kick.
