Tony, You missed two:
How about on a free throw when the ball is to remain dead following the throw, or if it happens on a try for goal following a foul.
I didn't miss anything. Nobody gets the ball in those situations. The play simply continues as it would had the ball not gotten stuck. It is of no consequence.
