In NY, we use the NCAA rules/mechanics set, and this is our TO procedure:
Long blast making the 'T' on the timeout side
Take the ball from whomever has it, put it on the table
Check with SK for correct # of timeouts
give quick CASUAL TO status to R1
At :45 2 quick blows to get both teams back out
give FORMAL timeout status to R1 - who mimics
THEN give ball back to serving team, handing game back to R1 with hand
the short blasts at :45 are quite sufficient...
