If I
KNOW a substitution has taken place, I will have the lineup card out. If the coach does not come to me I will walk toward the coach, usually eliciting an "oh yeah, right.." comment from him/her as he comes to give me his subs.
If I can help it I am going to prevent an unreported sub just like I am going to prevent an illegal sub i.e. coach wants to reenter a sub in a different spot in the batting order.
