A twist of the OP: Instead of the ball hitting the batter while she is completely out of the box, the ball hits her bat instead. Would you call her out for hitting a ball with her foot outside the box?

I actually had that happen in a Little League Minors baseball game (ironically, the batter was a girl at the time). The very slow arcing pitch (typical of LL Minors) was coming right at her, and instead of moving back out of the box, she moved across the plate. Problem was, she left her bat on her shoulder, and as she was clearly stepping on the plate with her foot, the pitch hit the top of her bat.
