Upon further review.

Even though it is not listed in the latest rule changes or included on the rules provided on line, the printed version of the ISF Rules does indicate that the batter must keep a foot in the batter's box between pitches (with the normal exceptions included in ASA), but there is no warning. The effect is an immediate strike on the batter, no pitch needs to be thrown.

However, even in the printed version, it is not included in the rules, but in what seems to be a Rules Supplement section and is highlighted as "Key Notes".

And here I thought NFHS & NCAA could be coy about where the insert rules/interpretations!!!
